Conformity Index Calculator

Enter the target and isodose volumes from your treatment planning system to get the common conformity and fall-off measures. Each one appears as soon as its inputs are filled in.

Educational SummaryAn original DoseLab explanation of a general concept. It teaches the idea; it is not a clinical recommendation.
Volumes (cc)

Read each volume from a structure in your planning system. Isodose volumes are the whole volume enclosed, including the part inside the target.

TVPIV is the overlap of the target and the prescription isodose structure (a Boolean AND). The 50% isodose is 50% of the prescription dose, not of the plan maximum.

      How it works

      CIRTOG (R100%) = PIV ÷ TV
      CIPaddick = TVPIV² ÷ (TV × PIV) = (TVPIV ÷ TV) × (TVPIV ÷ PIV)
      GI = V50% ÷ PIV   R50% = V50% ÷ TV

      The RTOG ratio comes from the RTOG radiosurgery quality-assurance guidelines (Shaw 1993) and is the definition in the NRG lung SBRT and brain SRS templates. It compares only the sizes of the two volumes, so an isodose the same size as the target but shifted off it still scores 1. The Paddick index (Paddick 2000) multiplies two shares — how much of the target is covered, and how much of the isodose lies inside the target — so it reaches 1 only when the two match exactly.

      The gradient index (Paddick and Lippitz 2006) and R50% (NRG lung SBRT and SRS templates) share a numerator, the 50% prescription isodose volume. The gradient index divides by the prescription isodose volume and so looks only at fall-off; R50% divides by the target volume and so mixes conformity and fall-off. ICRU Report 91 recommends the Paddick conformity and gradient indices for reporting stereotactic plans. See Conformity Index, Gradient Index and R50%.

      What this calculator does not do

      • No pass or fail. Acceptable values depend on the protocol, the target size and the technique — the NRG lung SBRT template, for example, tabulates R50% by PTV volume. DoseLab does not reproduce those tables; compare with the original protocol.
      • No location. Every index is a ratio of volumes. None shows where coverage is missing or where the fall-off is shallow; read the isodose lines slice by slice.
      • One target at a time. When the isodose clouds of neighboring targets merge, single-target values can no longer be calculated cleanly.

      Defines the gradient index (GI) as the ratio of the volume of half the prescription isodose to the volume of the prescription isodose — for a plan normalized to the 50% isodose line, the 25% isodose volume over the 50% isodose volume. The authors use it to compare plans of equal conformity, to explore prescription isodoses and to compare treatment modalities. Retrospective evaluation of SRS/SRT plan-quality metrics for arteriovenous malformations. Its methods section defines each metric: the RTOG conformity index (prescription isodose volume ÷ target volume; ideal 1), RTOG quality of coverage and homogeneity index, and the Paddick conformity index as the product of the under-treatment ratio (share of the target covered) and over-treatment ratio (share of the prescription isodose volume inside the target), noting that ICRU Report 91 recommends the Paddick conformity and gradient indices for reporting stereotactic plans. NRG Oncology Protocol Development Template for Radiation Therapy — Lung: SBRT
      Matuszak M, Chetty I, Robinson C, Weldon M, Timmerman R · NRG Oncology Medical Physics Subcommittee / Radiation Oncology Committee · 2016

      The template NRG Oncology gives investigators for writing the radiation section of a lung SBRT trial. Its imaging section allows PET/CT or MRI for structure definition, asks that the images be registered accurately to the planning scan (following AAPM TG-132 for deformable registration), notes that PET may be needed to separate tumor from atelectasis or fibrosis, and suggests an extra margin when a fused image is the primary image for the GTV. Its target and planning sections (5.2.4 and 5.2.6–5.2.8) define the plan-quality measures for lung SBRT trials: the prescription as the dose covering 95% of the PTV, a D99% requirement, R100% (prescription isodose volume ÷ PTV), R50% (50% prescription isodose volume ÷ PTV) and D2cm (maximum dose at 2 cm from the PTV in any direction, as a percentage of the prescription), with a table of allowable dose spillage by PTV volume. It describes a 20 mm PTV expansion structure used to control intermediate dose spillage, asks that dose above 105% of prescription be avoided outside the PTV, describes a deliberately heterogeneous dose with the hotspot in the central GTV, defines the maximum dose on a 0.03 cc volume, and ranks planning priorities when target coverage and organ limits conflict.

      NRG Oncology Protocol Development Template for Radiation Therapy — Brain: SRS
      Cui Y, Ogunleye T, Kirkpatrick J, Xiao Y, Sperduto P, et al. · NRG Oncology Medical Physics Subcommittee / Radiation Oncology Committee · 2016

      The NRG Oncology template for the radiation section of a brain radiosurgery trial. Its example text asks for CT slices of 1.25 mm or less, strongly encourages MRI (contrast T1 and a 3D gradient-echo sequence) for target delineation, recommends fusing FDG-PET/CT when available, and asks that the fusion be done by a medical physicist and reviewed by the attending physician. Its plan-evaluation section defines the conformity index (CI, or R100%) as the prescription isodose volume ÷ PTV volume, R50% as the 50% prescription isodose volume ÷ target volume ("R50% defines dose falloff outside the target"), a heterogeneity index D5% ÷ D95%, and coverage as the PTV volume receiving the prescription dose ÷ PTV volume, and notes that criteria such as the CI should account for target size and treatment technology.
