NRG Oncology Protocol Development Template for Radiation Therapy — Lung: Standard Fractionation
Matuszak M, Ritter T, Li Z, et al. · NRG Oncology Medical Physics Subcommittee / Radiation Oncology Committee · 2024

The NRG Oncology template for the radiation section of a conventionally fractionated lung trial. Its imaging section allows PET/CT or MRI for structure definition but asks that the final contours be confirmed on the planning CT, follows AAPM TG-132 for deformable registration, and notes that PET can help separate tumor from atelectasis. For plan evaluation it defines the prescription as the dose covering 95% of the PTV, with a protocol-specific D99% requirement, and asks that any dose above 105% of the prescription fall within the PTV rather than in normal tissue outside it.

Endorsed eviQ protocol for pre-operative short-course radiotherapy (25 Gy in 5 fractions) for rectal cancer. It notes that few published protocols give organ limits for short-course IMRT or VMAT, and takes its bowel and bladder values from the Royal College of Radiologists' national rectal IMRT guidance. It also asks for care in sparing the anal canal for tumors 6 cm or more from the anal verge. Its review was due 30 June 2026, so a newer version may appear.

A Story of Hypofractionation and the Table on the Wall
Timmerman R · UT Southwestern Medical Center · 2022

An editorial history of the UT Southwestern SABR constraint tables — often called the “Timmerman tables” — covering how they were developed and revised, and their relationship to AAPM TG-101 and NRG/RTOG protocols. Tables 2–11 separate serial tissues (listed with a volume, a volume max and a max point dose) from parallel tissues (listed with a critical volume and a critical-volume dose max). In Tables 2–7 a footnote defines “point” as ≤0.035 cm³; Tables 8–11 add a contouring-instructions column for each structure.

DoseLab read the tables in the authors' accepted manuscript (UCL Discovery and White Rose Research Online): Table 6 (intracranial, 1, 3 and 5 fractions) for the brain pages, and Tables 2, 3 and 5 (thoracic, abdominal and spinal) for the liver SBRT page. Each table gives optimal and mandatory limits by fraction count; the liver limits apply to patients with, at worst, Child-Pugh A6 liver disease. The kidney limit is written as a "cold" constraint — at least 200 cc must receive no more than the stated dose.

Heart toxicity from breast cancer radiotherapy: Current findings, assessment, and prevention
Piroth MD, Baumann R, Budach W, et al. · DEGRO Breast Cancer Expert Panel · 2019

Review by the DEGRO breast cancer expert panel, ending in recommended limits for the whole heart, the left ventricle and the LAD (Table 3). The authors state that the limits apply to left-sided breast radiotherapy, were developed mainly for whole-breast treatment, may be achievable with conventional and hypofractionated schedules, and may be unavoidably exceeded when the internal mammary nodes are treated. DoseLab read the values from the open-access full text.

AAPM task group report on the relative biological effectiveness (RBE) of proton beams. It defines RBE as the ratio of reference-radiation and proton doses producing the same biological effect, notes the ICRU recommendation that RBE-weighted dose be the product of absorbed dose and RBE (unit Gy(RBE)), and summarizes that RBE increases with LET and therefore with depth across a spread-out Bragg peak (on average about 1.1 at entrance, 1.15 at the center, 1.3 at the distal edge and 1.7 in the distal fall-off, for 2 Gy fractions and in vitro cell survival). It concludes that it is premature to adopt a variable RBE model clinically and that a constant RBE of 1.1 should in general be maintained, and lists planning methods commonly used to limit end-of-range RBE and range effects, such as beam angle selection to avoid ranging directly into an adjacent critical structure. The 6th R of radiobiology: reactivation of anti-tumor immune response
Boustani J, Grapin M, Laurent PA, Apetoh L, Mirjolet C · 2019

Open-access review. It defines the "five Rs" — repair, re-assortment, repopulation, reoxygenation and radiosensitivity — as the most important biological factors influencing tumor and normal-tissue response to fractionated treatment, and proposes a sixth R, reactivation of the anti-tumor immune response by radiotherapy, discussing how dose and fractionation may affect it. AAPM TG-263: Standardizing Nomenclatures in Radiation Oncology
Mayo CS, Moran JM, Bosch W, et al. · AAPM · 2018

Recommendations for standardized naming of targets, organs at risk and dose-volume metrics. Section 9 sets out the DVH-metric nomenclature DoseLab follows: Vx (volume receiving at least dose x), Dx (minimum dose to the hottest sub-volume x), CVx and DCx (the "cold" complements), output units in square brackets (Gy or % of prescription; cc or % of the structure), and an EQD2Gy unit with the α/β written in parentheses — for example D90%(10)[EQD2Gy]. Section 10 asks that DVH metrics say whether a lower-dose PTV is segmented (excludes the higher-dose PTV) or not. Read in the AAPM report PDF.

NRG Oncology Protocol Development Template for Radiation Therapy — Liver: SBRT
Parker WA, Sohn JW, Wolfgang J · NRG Oncology Medical Physics Subcommittee · 2018

The template NRG Oncology gives investigators for the radiation therapy section of a liver SBRT trial, based on NRG GI-003. It strongly recommends implanted fiducials, allows several motion-management techniques, and describes the multiphase contrast CT used for hepatocellular carcinoma: non-contrast, arterial phase (hypervascular tumor), portal venous phase (vascular thrombus) and delayed phase (washout). Contrast-enhanced MRI is recommended when contrast CT can't be done.

Review of proton RBE. It explains that proton prescriptions and organ constraints are based on physical dose multiplied by a generic constant RBE of 1.1, derived from early in vivo measurements; that RBE varies with LET, dose, biological endpoint and tissue radiosensitivity; and gives typical dose-averaged LET values across a spread-out Bragg peak, rising from about 0–3 keV/µm up to the center to 9–15 keV/µm in the dose fall-off, with the rise in LET and RBE extending the biologically effective range by 1 to 2 mm. Systematic review of 64 clinical studies reporting α, β and α/β for human tumors. It found large heterogeneity within and between studies; estimates for prostate and breast tumors generally indicated high fractionation sensitivity, those for head & neck, cervix, bladder and liver tumors low sensitivity, and others were mixed. It notes that whether the LQ model holds at large fraction sizes is debated, and recommends choosing parameters by site, histology and model and exploring a range of values. NRG Oncology Protocol Development Template for Radiation Therapy — Brain: SRS
Cui Y, Ogunleye T, Kirkpatrick J, Xiao Y, Sperduto P, et al. · NRG Oncology Medical Physics Subcommittee / Radiation Oncology Committee · 2016

The NRG Oncology template for the radiation section of a brain radiosurgery trial. Its example text asks for CT slices of 1.25 mm or less, strongly encourages MRI (contrast T1 and a 3D gradient-echo sequence) for target delineation, recommends fusing FDG-PET/CT when available, and asks that the fusion be done by a medical physicist and reviewed by the attending physician. Its plan-evaluation section defines the conformity index (CI, or R100%) as the prescription isodose volume ÷ PTV volume, R50% as the 50% prescription isodose volume ÷ target volume ("R50% defines dose falloff outside the target"), a heterogeneity index D5% ÷ D95%, and coverage as the PTV volume receiving the prescription dose ÷ PTV volume, and notes that criteria such as the CI should account for target size and treatment technology.

NRG Oncology Protocol Development Template for Radiation Therapy — Lung: SBRT
Matuszak M, Chetty I, Robinson C, Weldon M, Timmerman R · NRG Oncology Medical Physics Subcommittee / Radiation Oncology Committee · 2016

The template NRG Oncology gives investigators for writing the radiation section of a lung SBRT trial. Its imaging section allows PET/CT or MRI for structure definition, asks that the images be registered accurately to the planning scan (following AAPM TG-132 for deformable registration), notes that PET may be needed to separate tumor from atelectasis or fibrosis, and suggests an extra margin when a fused image is the primary image for the GTV. Its target and planning sections (5.2.4 and 5.2.6–5.2.8) define the plan-quality measures for lung SBRT trials: the prescription as the dose covering 95% of the PTV, a D99% requirement, R100% (prescription isodose volume ÷ PTV), R50% (50% prescription isodose volume ÷ PTV) and D2cm (maximum dose at 2 cm from the PTV in any direction, as a percentage of the prescription), with a table of allowable dose spillage by PTV volume. It describes a 20 mm PTV expansion structure used to control intermediate dose spillage, asks that dose above 105% of prescription be avoided outside the PTV, describes a deliberately heterogeneous dose with the hotspot in the central GTV, defines the maximum dose on a 0.03 cc volume, and ranks planning priorities when target coverage and organ limits conflict.

Open-access review of tumor hypoxia in radiotherapy. It traces the history from the early 1900s to Gray's work in the 1950s, describes chronic (diffusion-limited) and acute (fluctuating blood flow) hypoxia, reviews attempts to modify hypoxia — including nitroimidazole radiosensitizers, where nimorazole in head and neck cancer is the only one incorporated into standard treatment, and only in Denmark — and notes that although OER falls as LET rises, the oxygen effect only disappears at around 500 keV/µm or greater, beyond what can be achieved clinically. Open-access modeling paper on the physical basis of the oxygen effect. Its introduction summarizes the oxygen enhancement ratio: relative to anoxia, well-oxygenated tumors respond better by a factor of 2.5–3; half-maximum radiosensitivity occurs around an oxygen partial pressure of 3 mmHg, with maximum OER typically reached above 20 mmHg; and oxygen must be present before irradiation or within microseconds of it. It explains the oxygen fixation hypothesis — that DNA radical damage can usually be repaired, but repair is more difficult or impossible when the radical has reacted with oxygen. Full text read in Europe PMC (PMC4765087). (The journal classes it as a research paper; DoseLab uses it for its introductory summary.)

Randomized trial of 40 Gy in 15 fractions versus 50 Gy in 25 fractions for loco-regional breast radiotherapy. The protocol lists the DBCG organ limits used in the trial (Table 7) and quotes the DBCG guideline LAD maximum of 17 Gy (hypofractionated) and 20 Gy (normofractionated), noting that heart limits were under review because of the benefit of treating the internal mammary nodes — so the trial asked centers to delineate the LAD and report its dose rather than enforce a limit.

Definition of stereotactic body radiotherapy: principles and practice for the treatment of stage I non-small cell lung cancer
Guckenberger M, Andratschke N, Alheit H, et al. · DEGRO Working Group Stereotactic Radiotherapy · 2014

DEGRO working-group statement defining SBRT for stage I NSCLC. It lists the motion-management approaches in routine use — free-breathing delivery with the internal target volume concept (described as the most commonly practiced), the mean target position concept or real-time tracking, and delivery in specific breathing phases by gating or breath hold — and names safety margins for critical organs (the PRV concept), repositioning or replanning as ways to avoid organ overdose. Review of the radiobiology behind SRS and SBRT. It lists the five Rs — repair, repopulation, redistribution, reoxygenation and intrinsic radiosensitivity — notes that the first four were described by Withers and the fifth added by Steel, and explains that redistribution and reoxygenation increase cell kill over a fractionated course while repair and repopulation increase cell survival. It argues that hypoxia is likely to matter more for SRS/SBRT than for conventional fractionation, and concludes that for most tumors the classic 5 Rs and the linear-quadratic model are sufficient, with the excellent clinical results explained by the much larger BEDs delivered (with the likely exception of enhanced antitumor immunity for some tumors). AAPM task group report on biologically based treatment planning. It describes the common models — the Poisson TCP model, empirical sigmoid TCP models defined by D50 and a normalized dose-response gradient, and the Lyman-Kutcher-Burman NTCP model with parameters TD50, m and n — defines serial and parallel organs and the volume effect, and advises using the LQ-equivalent dose in 2 Gy fractions rather than physical dose in TCP and NTCP calculations. Its recommendations: plan evaluation should at present be based on established dose-volume criteria; relative TCP/NTCP estimates may help choose between rival plans if users understand the models and parameters in their planning system; absolute TCP/NTCP estimates are not warranted as the main indicators of plan quality; and review of the 3D dose distribution should always remain part of plan evaluation. Read in the AAPM report PDF (Report No. 166, March 2012).

Review by the originator of the term biologically effective dose (BED, introduced in Br J Radiol in 1989). Gives the BED formula — nd[1 + d/(α/β)], with an optional overall-time term — and the conversion to the equivalent dose in 2 Gy fractions by dividing BED by 1 + 2/(α/β) (1.2 for α/β = 10 Gy, 1.67 for 3 Gy, 2.0 for 2 Gy). Its worked example: 30 × 2 Gy = 60 Gy is 72 Gy10 to early-responding tissue and 100 Gy3 to late-responding tissue, but 60 Gy EQD2 to both. Full text read in Europe PMC (PMC3473681).

Defines the gradient index (GI) as the ratio of the volume of half the prescription isodose to the volume of the prescription isodose — for a plan normalized to the 50% isodose line, the 25% isodose volume over the 50% isodose volume. The authors use it to compare plans of equal conformity, to explore prescription isodoses and to compare treatment modalities. AAPM task group report on respiratory motion in thoracic, abdominal and pelvic treatment. It groups management approaches into motion-encompassing methods, respiratory gating, breath-hold techniques, forced shallow breathing (such as abdominal compression) and respiration-synchronized (tracking) techniques. Read in the AAPM report PDF (Report No. 91, July 2006): using ICRU Report 62 nomenclature, the PTV adds margins to the CTV for respiratory (intrafraction) motion, interfraction motion and set-up error; without a respiratory management device the entire range of motion should be considered in the internal margin; and for lung tumors a maximum intensity projection (MIP) can give the motion-encompassing volume, provided there is no mediastinal involvement.

Defining the tumour and target volumes for radiotherapy
Burnet NG, Thomas SJ, Burton KE, Jefferies SJ · 2004

Review of the ICRU target-volume concepts. It summarizes ICRU Report 62's internal margin (for variations in the size, shape and position of the CTV, for example from organ filling or respiration) and set-up margin, and its planning organ at risk volume (PRV): a margin around an organ at risk, analogous to the PTV margin, because systematic and random errors apply to organs at risk as much as to the CTV. It notes that a PRV substantially enlarges the organ and may create dilemmas with target dose, so it is needed only in certain circumstances — around organs whose damage is especially dangerous, such as the spinal cord, a "serial" tissue. NRG Oncology's protocol page for RTOG 1005, a phase 3 trial of hypofractionated whole-breast irradiation with a concurrent (SIB) boost. The protocol document holds the trial's dose–volume requirements, set separately for each arm: Arm I (50 Gy in 25 fractions, sequential boost) and Arm II (40 Gy in 15 fractions, concurrent boost to 48 Gy). Both arms are tangent-style whole-breast treatment without nodes, and skin bolus was not allowed. DoseLab checked the 50 Gy / 25 fx ipsilateral lung limits against section 6.4.3.5 of the 24 May 2011 version.
